Please note, compared to the online application system, the paper form will require a longer processing time as it involves manual input from the Applicant, Medical Practitioner and the Council. This may result in a possible delay on the permit outcome.
If the Applicant does not have a have a mobile phone, they may be able to use a family member or carer’s mobile number. This is where the application reference will be sent to, as well as any other updates about the application.

The applicant will receive the paper form via council and after entering some personal details, they will then take the form to their next medical practitioner appointment (GP’s and occupational therapists only).

The medical practitioner will complete the medical assessment, then the applicant will take the completed form to council.
